Adding Multiple Devices:

You can use this form to add up to five devices. Here's how:

1. Fill in the table for Device 1.
2. Click the drop-down Device list and select another device.
3. Fill in the table for this device.
4. Repeat these steps for up to five devices.

Before submitting the form, you can return to any previously-entered information by simply choosing that device from the drop-down list.

• Is this a move? If the device has been connected to the 4J network before, answer "Yes" to this question.
• If you answered "Yes" to question (8), you must select an old school or building in (8a). (Old Building is the place the computer is being moved from.)

• Type -Phone, Printer, Server, or Wireless? Do not register Desktop computers.
• A Hardware address (ethernet address) is exactly 12 characters long (no colons, please). An example is: 0800fcbedd00
• For wireless Mac laptops enter the "Airport" address.
• Building and Room refers to the location of the device being registered.
• For the Delta Plate information, include the ENTIRE label on the wall plate. An example is: A1-4-D12
  *For wireless devices please enter the wall plate numbers from the main office.
• If you do not have a serial number just enter "NA" and complete the form.
